Music Organizations Manages and Pays Verification

This template is used when the MCMO wants to pay for all of their Members. MCMO to provide Member required info highlighted in green for each requested domain name. If MCMO wishes to take on initial management of their Member's domain name, the MCMO can fill in the MCMO's legal address and phone instead of the Member's legal address and phone. If a Member is a business or an organization, provide legal business or organization name, instead of first and last name.
MCMO must provide legal name of Member and email, as this information is required for identity verification by the Member. Later on, my.MUSIC can work with the MCMO to update the MCMO information with the Member's information.
Optional information are highlighted in gray and represent proxy contact (admin). Upon submission of this list, my.MUSIC will send confirmation email with invoice payment information. Please delete the sample data (highlighted in yellow/gray) before submitting.
